Living with Chronic Health Conditions

If your health condition has persisted for more than six months an experienced team of doctors, nurses and allied health professionals can work in conjunction to support you.

Coordinated care can be provided through Health Assessments, a Chronic Disease Management Plan and Team Care Arrangements.

These services have significant Medicare rebates.

 


 

What is a GP Chronic Condition Management Plan? (GPCCMP)
A GP Chronic Condition Management Plan (GPCCMP), is a bulk billed comprehensive written plan developed by a General Practitioner (GP) or nurse for a person with a chronic or terminal condition. It outlines health and lifestyle goals, patient actions, and necessary treatments, services, or referrals to allied health professionals. 
The GPCCMP aims to improve care continuity and simplify the management process by replacing previous GP Management Plan’s (GPMPs) and Team Care Arrangements (TCAs).
 
What is involved?
Your GP can review your health conditions to see if you are eligible for a GPCCMP.
For an initial GPCCMP, the appointment takes approximately 30 minutes with a nurse, followed by a 15-minute consultation with your GP. Our nurses can provide support and monitoring between visits to your GP if required.
GPCCMP reviews are usually booked in with your regular GP for a 15-minute appointment at a 3, 6 or 12 monthly interval, depending on your medical condition and/or needs. This can be discussed with your nurse and/or GP. We recommend regular reviews to ensure your goals are being met and to see if any changes may be needed.
GPCCMPs are intended to be provided by your usual GP or practice.
 
Referrals to Allied Health Services
Referrals made to Allied Health Professionals  for Medicare rebates can include;
·         Aboriginal Health Workers or Aboriginal and Torres Strait Islander Health Practitioners 
·         Audiologists 
·         Chiropractors 
·         Diabetes Educators 
·         Dieticians
·         Exercise Physiologists
·         Mental Health workers
·         Occupational Therapists
·         Osteopaths
·         Physiotherapists
·         Podiatrists
·         Psychologists
·         Speech Pathologists 
Visit entitlement is   5 services per calendar year as tallied by Medicare

Medical Cannabis Consulting

Dr Nish Balakrishnan, a general practitioner consulting from the Rosebud clinic is also a certified medical cannabis prescriber.

Consultations are based on best medical practice and Australian clinical guidelines. Prescriptions are not guaranteed. There are strict TGA rules around medical cannabis prescribing. Generally, you are only eligible if:

1: You have a chronic medical issue (which can be treated with medical cannabis) that has been present for 6+ months

2: You have tried other treatments which been ineffective or have produced undesirable side effects or are unsuitable for you.
